Your time frame to qualify for enhanced benefits
First year members
At the start of the plan year, you’ll automatically receive enhanced benefits for the first 90 days. You must meet the requirements within the deadlines to keep enhanced benefits. If you don’t:
- Everyone on your health care contract moves to standard benefits on the 91st day of your plan year.
- You’ll remain with standard benefits throughout the plan year.
Continuing members
At the start of the new plan year, you may have enhanced or standard benefits:
- If you have enhanced benefits, complete the requirements to keep them. If you don’t, you will move to standard benefits 91 days from the start of the plan year.
- If you have standard benefits and meet the requirements, you will move to enhanced benefits 91 days from the start of your plan year.
Note: If your doctor noted on your Qualification Form that your follow-up visit will be 13-24 months, you don’t have to send us another form or take the health assessment this plan year. Exception: If you smoke, you must join Quit the Nic within 120 days and actively participate until you quit smoking.
