How to take a health assessment
- Log in to Member Secured Services. If you have not already done so, you will need to register. Your health plan ID card has information you’ll need.

IMPORTANT: Each member taking the health assessment must log in with their own ID and password for us to have a record of the completed assessment. - Click the health assessment link.
- Complete the health assessment and you'll receive your lifestyle score and a personalized plan that includes what you're doing well and what you can do better.
To find Nourish®, Balance® and other health questionnaires
After you complete the health assessment, click on the Home tab. Look under the My Health Programs tab.
If you have technical difficulty with the Balance questionnaire, please call 1-888-772-7147.
