How to take a health assessment
- Log in to Member Secured Services. If you have not already done so, you will need to register. Your health plan ID card has information you’ll need.

IMPORTANT: Each member taking the health assessment must log in with their own ID and password for us to have a record of the completed assessment.
- Click the health assessment link.
- Complete the health assessment and you'll receive your lifestyle score and a personalized plan that includes what you're doing well and what you can do better.
To find Nourish®, Balance® and other health questionnaires
After you complete the health assessment, click on the Home tab. Look under the My Health Programs tab.
